Survey on Families Experiences with Schools related to Mental Health and Special Healthcare Needs

  • Family members of children with mental health, substance abuse, or special healthcare needs are being invited to participate in a survey that will inform multiple statewide and local community projects.  The survey will help understand how Colorado schools, health care providers, and mental health care providers are meeting the needs of kids.  This survey asks questions to parents of children ages 0-18 with any physical or mental health issues, and can be opened by clicking the link below.  The survey is open until September 30th, 2008: http://www.surveymonkey.com/s.aspx?sm=Zh0cpfslnoN6mfUCWrDHVQ_3d_3d
